Medical errors can lead to medical malpractice lawsuits

Medical experts cited some common mistakes in charting, such as the failure to adequately record a patient's health history or drug information. The response to current medication should be noted in the charts as well, to determine if there are any adverse reactions. Orders written in the charts are sometimes transcribed incorrectly, resulting in wrong dosages of medicine or improper treatment. Finally, records may be illegible or incomplete.

Doctors, nurses and other healthcare personnel are not infallible and may make a mistake in a patient's medical treatment.
