Prescription errors vary but all can cause patient harm

In one of our blogs last week, we discussed how important it is for doctors to take a variety of considerations into account when they are prescribing medications. These considerations include known medical conditions, other medications being taken and the medical diagnosis, as well as others. When a doctor doesn't properly factor all of those considerations into a prescription choice, the patient can suffer serious harm.

When a doctor decides on medications to treat a medical condition, the doctor has to carefully weigh the risks of the medication with the benefits of treating the condition. In some cases, this can be a very difficult decision, but that doesn't mean that any patient should suffer harm because of the decision that is made.

If there are possible interactions or dangers associated with the medications that a medical professional chooses, the doctor should discuss those with the patient. That gives the patient the chance to accept or decline the medication. This concept is known as informed consent, which is a vital concept in patient care that should never be bypassed.

In some cases, incorrect dosing instructions might occur because the doctor wrote the prescription in a manner that makes it difficult to read or because the doctor gave the incorrect instructions. In either case, patients who take the medications as directed might suffer harm.
